In emergencies, we also case manage post-injury patients seen at the Our Lady of Fatima Hospital Emergency Department

RI Army National Guard

For all Rhode Island Army National Guard personnel scheduling an annual physical exam, please print out and complete the attached questionnaire and bring with you to your appointment.

Please Note:
You must fast for at least 8 hours prior to exam.
Anyone who wears a respirator must bring it with them.
